Sports Physical Information

 

SPORT REQUIREMENTS FOR STUDENTS:

The policy requires all athletes to have a current physical on file with the nurse prior to trying out for any team. 

Physicals are valid for 13 months from the date of the exam. If the physical will expire during the participant’s season, a new physical is required prior to the athlete trying out, practicing or playing in games. It is highly recommended to obtain the physical by your private physician. 

The blue State of Connecticut health assessment record includes a section for sports clearance. If the student goes to her private physician for a sport physical, this is the form to be used. This form must be completed and on file in the Mercy High School Health Office prior to tryouts.
